To be eligible for election, this form with original signature (no copies) MUST be in the hands of the Association Secretary prior to the first business meeting at the 2025 Conference. If mailed prior to the Conference, the form must be mailed directly to the Association Secretary, addressed as printed below. To ensure its arrival, send by CERTIFIED mail, return receipt requested. Statements of Intent received after the opening gavel of the first business meeting will be automatically disqualified.
